Black Box In Screen

when I turn on the tv the picture is fine but when I change the chanel this solid black box appears in the center of the screen. If I hit the menu button the box goes away until I change the chanel then again it pops back on the screen. Help please advise

turn the closed captions off "text". hit the "CC" button on the remote til it says "off".
