Refrigerator Air Vent Iced Over In Refrigerator Section

We noticed that our refrigerator seemed to be running warm even though temp settings for refrigerator were set at 32 degrees. (The milk was too warm for our liking in particular). Upon further inspection we found a large quantity of ice had formed over the air vent on the rear panel (bottom) of the refrigerator. We had to shut it off to chip away at the ice. What caused this? Now that we removed the ice is it safe to just run it again?
