Straight Talk Unlimited Card Activation

I bought a Straight Talk Unlimited Card from Walmart yesterday. When I tried to scratch the strip, a piece of the paper tore and I cannot see the first 3 digits. I tried to activate the card but was prompted that I need 15 digits. I only have 12 showing. When I was given my receipt the cashier pointed to a number at the top of my receipt where it says card #. How can I use that as proof of purchase. My phone was disconnected as of this morning and I really need to activate the card.
