Facebook assures that Friend removals are not caused by technical errors. The issue may result from one of the following:

This person removed you from his/her Friend List, either intentionally or accidentally.
This person has deactivated his/her account, causing a disappearance from all Friend Lists.
It's also possible there may be a security issue.
