Code Card

I am trying to get a replacement code card for my Zenith ZC800 universal remote control. The card came with the remote control along with the manual when I first purchased it. I still have the manual but lost my code card. I have searched online everywhere but have not had any luck locating one. Someone please tell me where can I find one?
