First, request a password reset, by clicking the "Forgot your password?" link located near the "Login" button on the login page. Facebook will then send per your instructions a link to reset your password. (This information is from this Facebook page, which may provide additional guidance.)

Then, send Facebook information about the account here so Facebook can look into the issue and take the necessary steps.
