Submit the dialog beginning on this Yahoo page. (If you don't know your Yahoo ID, start with the second option.)

You can recover your Yahoo account using your alternate address instead of your security questions, as explained here. Or you can try phoning 866-562-7219, 408-349-3300, or 866-850-4303.
