My Xbox One Will Occasionally Not Read Games. What Is Causing This?

The majority of the time when we put a game into the console, the console will not read the disc. Occasionally it will work, but it is few and far in between.

You can try following the troubleshooting steps here but more than likely there's a problem with the disc drive. You may need to send it in to Microsoft for repair.
