The Defrost Hole Plugs Up And I Get Water In The Refrgerator And Under The Crisp

this happened just 2 weeks ago and then happened again this past weekend. This is the 3rd time we have had this problem. We have defrosted the freezer and gotten water to run through the hose and have checked that the freezer fan is running and it is. How can we prevent this from happening again?
