This is a DLP rear projection HDTV that uses an optical engine to produce the picture. The selected input video signal is amplified and modified before it's applied to the multi-million mirrored device (the DLP chip) that moves these mirrors to show a representative of the the picture (the red, green and blue components dependent upon the position of the color wheel). So when these mirrors stick the resulting error is either a white or black speck. The only way to replace this chip is to replace the entire engine: Manufacturer   Toshiba Part Number 75006693 Description OPTICAL BLOCK, 95.   List $410.00

after the old one is sent back for the $220 core. and in stock here: http://www.andrewselectronics.com/welcome.aspx

figure the tech will charge $200-$250 in the shop to replace.

No, the spots/specs are not going to go away. If they appear to be about the same size, it is likely that the optical block is going bad. This is an expensive repair (guess about  $450 for the part and another couple hundred for the tech service). If they seem to be all different sizes and shapes, it could be that the unit needs to be cleaned.

No,these are dust smudges that builds up inside on the surface of the screen. It just a matter of having the technician clean your screen and that will fix his problem.
