On Hold

I receive a call and answer it. The phone goes into line 1 on hold mode and I can't talk to the person. I hit resume and I can't hear the phone caller. I made calls the other day and it seemed fine. It's when some one calls me that this happens. I don't know if I'm hitting the volume button by mistake and it goes into this mode or I'm grabbing the phone wrong and it goes into this mode. Why does it do this? How can I prevent this from happening again. I'm missing calls.
