Enrolling In Dental Plan Offered To Glaxosmithkline Retirees.

My late husband was an employee of GlaxoSmithKline and they are offering a one-time dental plan for retirees and their spouses. I would like to have more information such as cost, services. Are you limited to choose a dentist that carrys your plan or would I be able to use my current dentist? Thank you.
