Payable On Death Account Distribution Request

My mother passed away in Nov 2014 and has listed my brother and me as beneficiaries of several payable on death accounts. He lives in Florida and I live in CA. I have serious medical issues which do not allow me to travel to a branch to be identified. My brother said the 5/3 branch near his house in Palm Beach Gardens, FL indicated that I could obtain a notarized letter from my bank which he can bring in with the certified death certificate to obtain distribution of the payable on death accounts. Question: Do you have a form that my bank can notarize for him to bring into the branch? If not, is there an example available of what the letter should say. To whom should the letter be addressed? Will I be issued a check or can I ask for a wire transfer to my CA bank account?Thanks for your help, Joe
