You need to allow approximately 8-10 weeks for the Rewards Card to be delivered.

tried with the website www.boostrebates.com? There you can submit the rebate, or fill in the Mail In Rebate form received from the dealer and mail it to: Boost Rebates Dept 17-16850, P.O. Box 540017 El Paso, TX 88554-0017

Remember that you have to allow approximately 8-I10 weeks for the Rewards Card to be delivered. You can check the status by going to www.sprintrebates.com
