Obviously, your SD card failed to function properly. This problem requires replacement with the brand new one. You can purchase SD memory cards online at Amazon.com. Here's the link to the SD memory card products.

hello man.... does your sd card is showing blank on mobile only or even when you connect it to pc... if it is blank even when connected to pc... then probably a virus has formated your sd card. yes there are virus in phones also.. then try to put some data on your sd card and load it on your phone and see if it still doesn't then your sd card has reached its limit and you will need to buy one
