Why Do You Keep Going Up On Your Rates?

My husband & I are both retired & disabled and it is hard to sometimes hard to make the deductibles.  We are on a fixed income & have to cancel some appointments because we cannot afford the deductibles, or the co-payments on prescriptions. Yet you keep going up on your rates.  I'd like to know why?
