Greetings - A PUK (Pin Unlock Key) is prompted for when a Sim card's Pin code has been entered incorrectly 3 times. To obtain the PUK you will have to call the Carrier's Customer Service line (if you are fortunate, you may only have to listen to the voice prompts and simply select the PUK option from the menu without having to speak to a live person. You will be expected to know the cell number of the Sim card.

Hope that is helpful.
