I've had to do this twice now. You can do it online, there is a form but it's burried within the Yahoo web site and I've lost the location.

The best and most efficient way is to wait until the AT&T account is terminated. Then after you log on to your Yahoo account you'll get a notice that will ask if you want to un-merge.
