Bottom Drawer Off Track

The bottom drawer (where pans are kept) has come off track and I cant get it back on track. After getting it out I found two pieces that had come unscrewed. I can't find a place for them or how or where they should be attached. My drawer has never really hung correctly. It's always given us some problems. I need instructions, pictures, manual or video to help with this. Thanks so much
