A Smooth, Curved, White Chunk Of Plastic Has Come Out Of Ice Dispenser, Twice!

This piece has all smooth edges except for a jagged one on one side. This is the second time this has happened, the first time it was dismissed, couldnt figure out how it got there. Please explain what is happening here and why. What can be done about it? Someone can choke on this.......

Hi. The explanation here by Applianceman(both post) should be able to account for the broken pieces found in ice cubes.
