Health Insurance Card

I am employed by Youth Villages and have ben enrolled in the health since 05/01/11 snf hvr yet to receive an insurance card. I have checked with my representative and have been told that the card is in the mail on two different occasions. Is ther any way i can get a temporary card to verify that i have insurance. YV's group number is 704966. Thanks for your assistance

The fastest way to order a replacement ID card from United Healthcare is to use their www.myuhc.com web site. Here is some more information as well:

Here are the instructions from the myuhc.com web site for registering if you do not have your ID card:
- If you don't have your ID card AND:
  - You already have a user ID and password, you can request new ID cards and print temporary ID cards online. Just log in and click Account Settings at the top of the screen.
  - You don't have a user ID and password yet you can register with your Social Security number and birth date. If you do not want to provide this information; please call us toll-free at 1-877-844-4999 for help. We are available Monday through Friday, 8:00AM-10:00PM ET.
  - You already have a user ID and password are having trouble logging in and don't know your Member ID and Group/Account numbers, please call Technical Support toll-free at 877-844-4999 for further assistance. Technical Support is available Monday through Friday, 8:00AM-10:00PM ET.
