Brake Failure.

PEDAL TO FLOOR. NO BRAKES! GRADUALLY CAME BACK. BRAKE FLUID OK NO LEAKS. ALL PADS LIKE NEW. 2 ND EPISODE .DANGEROUS!

Your master cylinder is going bad. That's where you add the brake fluid. They wear out, but usually don't leak. But if you check under the dash on the driver's side, you may see some brake fluid. 

Not to hard to replace, but you have to know what you are doing. Read the instructions that come with the new master cylinder. Here's what your master cylinder looks like:
