Accident By Sf Holder How Do I File A Claim?

I was rear ended by a State Farm Policy holder, what claim form do I use?

Have you already resolved this?  If not, you can call a local agent and they can direct you to the correct phone number to call to get the SF insured's agents name and number.  Then, file it with that agent.
