Pedal Issues: Pedal Falls Off After One Ride Despite Tightening.

How do you fix a bike pedal that fell off?

A pedal that falls off is usually a sign that the pedal and the crank arm threads are severely damaged. The pedal should be replaced, but the crank arm threads can be repaired by reaming out the existing threads and tapping in a new "helicoil". The toolkit for this is fairly expensive and it takes skill to do it right, so it is usually not worth doing this yourself and usually only worth having a bike shop do this for a high-end crank arm. For lower-end cranks, it's usually better to just replace them. In the middle range (e.g., Shimano 105 or even Ultegra, and equivalent levels in other brands) you should probably decide whether it's worth it based on how old the crank is.
