Here they are:

Touch Menu and then go to  Settings. Touch Phone and finally touch Reset Phone.

I reset my phone the other day. I called StraightTalk because my screen seemed out of alignment. The customer service rep had me reset my phone to the factory settings. She had me go to menu, settings, reset phone & then I had to put in the code 2222, then I had to hit reset phone again & put in 2222 again. It worked for me somewhat. Good luck.
