If the DVD was broken or lost while in your care, you can pay for it by going to the Queue and clicking on the Report Problem link next to the disc, then choosing "I have lost or damaged a DVD and would like to pay for it." Replacement cost is $14 ($20 for Blu-ray).
If a large number of lost DVD reports are filed for an account, the account will be placed under review and the customer will be notified via email.
