I NEED THE PUK CODE ON MY LG400G CELL PHONE. CURRENTLY, THIS CELL PHONE SAYS EMERGENCY ONLY, READING SIM AND CHECK CALL RESTRICTIONS 5.

Go on their facebook page and explain your problem.  They will email you and try to help.  But don't put your phone number out where the public can see it.  They'll contact you privately for the info they need.
