Once at the link, there are instructions provided on how to correctly file your claim. Your wireless provider requires you provide a 'claim number' when you file.
To aquire this 'claim number' you must fill out the 'Claim Affidavit' which you can aquire with complete instructions by clicking on the "To view this affidavit" link on the webpage.
After your Claim Affidavit form has been received and processed, a Customer Service Representative will contact you with the claim number.
Then you can go back to the webpage to enter your phone number with your claim number, and finally file your claim for a replacement or repair.

Please keep in mind the deductible will have to be paid at the time you file the claim.
