Someone Has Hacked My Metro Pcs Phone. They Have My Info To My Phone.

someone has hacked my metro pcs phone. they put a screen lock on my phone.my calls are forwarded to their phone. how can i stop that? how can i get back to using my phone and make sure they have no access to my phone and information to do it again. i took my sim card out and when i call my phone it rings and then go to my voicemail.
