According to magicJack, you must return your equipment and follow this procedure to cancel:

Before returning equipment you must obtain a valid RMA number by visiting: http://my.magicjack.com/returns.html The procedure outlined there will get you the address to which to return your device.
All returned equipment must be in the original packaging with the UPC or bar code intact.
All components must be included.
