Ask For Rebate Of Thanksgiving Promottion

I did get my rebate back since Dec 3, On Nov. 29, I bought 2 blender coz Macy offreed Thanksgiving promotion. I checked out for getting a receipt and another duplicate receipt. The counter asked me to send them separately to get rebate. So far, I only get one. Where can I ask for the rebate? What telephone number? Or I want to refund the blender aASAP
