The first thing to realize is that Kaiser coverage will vary from region to region so to provide a phone or fax number would require that you specify your area.

The next thing is that although you complete the form for your self or for a family membership, that form is conveyed to Kaiser Permanente by the employer who has that information.

Application forms are available online but the Group is either an employment group or other such group if there is a group number. Please check with the sponsoring group about the proper procedure.

Just having access to the group number would not satisfy Kaiser that you are actually a group number. And of course, there is also the enrollment period to consider. That period can vary.

I'm sorry not to supply a number but there are several across the country. And Kaiser makes it quite clear that the form must be submitted by the employer/group and not an individual.

Should you simply fax a form to that office, I think it would be ignored so please go through the proper channels.
