Need To Send Mother's Poa By Fax To Humana. What Do I Include On Cover Letter?

Hello, you will need to contact Humana directly to obtain an exact answer, but information that you would probably want to include would be:
- the account number
- your name and your mother's name
- who the POA is being sent to (individual or department)
- your phone number and contact information
You may also need to send an Appointment of Representative Form, which you can download here.
