How Do I Get In Contact With Our Provider Representative?
The number I call brings me to Customer Service based out of Phillipines and I cannot get anyone there to process my claim. I want to file a ccomplaint for breach of contract.

No matter where the customer service unit is located you of course are entitled to responsible service. You may write to the corporate office at 500 W.Main St., Louisville, KY 40202. If necessary phone the Ketucky Office of the Attorney General, at 502-429-7134.
