Tl8130a1005 Low Voltage Output

I have 2 TL8130A105 line level thermostats. One controls a baseboard heating unit. The other controls a fan forced unit. The one on the fan forced unit worked fine for about 2 months, then started outputting very low voltage (10-40v ac) on the load outputs. I checked the line input (240VAC) and rewired the unit to bypass the thermostat and all is well. Is this a product failure or could it be something I did wrong?
