Hp 9250c Digital Sender Error

We have HP9250C Digital Sender with latest firmware upgrade that gives the following error, "A File name is required." The web interface shows that there is a file name in the required field. We have also put in a file name manually but whenever we press start button to scan to e-mail a document we continue to get "A file name is required" error. We have restarted several times, did cold reset, swapped out board, hard drive and nothing will get rid of error. Any ideas?

Doesn't make much sense but found the issue which was loose connection on pull-out keyboard. Once connection was re-seated the error went away.
