My Upper Spray Arm Keeps Falling Off.

tube threads coming though the top of the machine are NOT stripped.

From my experience, if the spray arm assembly keeps falling off and is not stripped or broken, or has missing parts, then the loading of the dishes might be touching and knocking the sprayer arm part(s) off.
