How To Replace A Front Driver Side Door Upper Hinge?

My hinge broke, in half. I think the bottom hinge is ok. I ordered a new one and hope it comes with a pin and bushing but realize it probably wont. I need tips and help. How do I get the buts behind the fender? Also its a stepside sport. 2002 f150 sport, 4.2 v6, single cab.
