1. Facebook wants you to send it information about the hacked account here so Facebook can look into the issue and take the necessary steps.

2. If you can log into your account, you can permanently lock yourself out of it if you press the Submit button on the deletion page. If not you have to first wait until your account is recovered (point 1 above).
