Password-reset Everytime I Log On, Then Don't Work On If I Log Out,same For Cell

when I log on to home pc to fb I have to reset it each and everytime I log in on . then if I log onto to cellphone says its not right password.i cant get the two to coordintaete been this way for two months nowcould someone have hacked my account if so would I have to deactivate the one I have and make new fb pagealso if I wANT to change email name from what I have now how do I make a change

if you got a Facebook password reset email that you didn't request, it's likely that someone accidentally entered your email address or username when attempting to log into their account.
