How Can We Report/block An Unauthorized Account That Was Created On Fb?

http://www.facebook.com/profile.php?id=100000876146508 this profile has been created without consent. After sending an email explaining this is identity theft and it was going to be reported it was slightly altered. Attempts have been made to report this account using the link provided by Facebook, but it doesn't seem to be working properly. Please advise!!!
