Remove My Child From Facebook

My child has been on Facebook for a period of 6mths. She is only 12yrs old. I don't know what the URL is and am not sure of her e-mail address. How do I have her removed? She is miss using the Network to post hateful messages to people that are not her friends on Facebook. Thank you.
