Keys Mysteriously Locked In A 2006 Pacifica

I seem to accidently locked the keys in my new wifes 2006 pacifica.  here are the facts:  1.  My wife leaves here keys partially in the ignition every night. 2.  I went to remove an item from the trunk and it would not open, so I went to the drivers door and hit the door unlock button to release the trunk. 3. I closed the drivers door, opened the trunk, removed the item, and closed the trunk. 4.  The next morning the doors were locked and her keys were locked inside. HOW DID THIS HAPPEN?  BESIDES REMOVING THE KEYS EVERY NIGHT, WHAT COULD I DO SO IT DOES NOT HAPPEN AGAIN.
