All radiator hoses use clamps to keep the pressure on the system from leaking. Your motor mounts may be allowing the engine to move enough that the hose gets into the fan. Replace the hose making certain that its routed to stay as far away from the fan as possible. Get someone to hold the brake & put the vechicle in drive to torgue the engine so you can check engine movement. If the motor move toward the fan then you know to have your mounts checked.
