Thread Getting Stuck

hi there im having trouble at the second thread holder  where it comes from a diagram on the top it is a like a spindle it keeps becoming too tight as it starts up the machine we have taken it a part to see it is spring loaded i am really confused as on a big stitch it works perfectly can you help please
