Re-boost Pre-paid Quote

I need assistance obtaining a quote from Boost Mobile for my next 6 months pre-paid Re-Boost phone service to include the cost of insurance. Upon my initial activation of my service, the Boost store I went to who had initially provided me my original quote, has closed. My reason for needing a written quote is that I am a paralyzed veteran who has a trust fund company pay my bills for me. In order for me to continue service with Boost Mobile is for me to obtain a written quote stating the monthly cost for phone service including insurance for the next 6 months, so I can submit it to my trust fund company and they will issue a check.
