I Lost My Anthem Medical Insurance Card And Need To Look Up My Coverage On Line

I HAVE A NEW JOB WORKING AT SPRING HARBOR HOSPITAL AND LOST MY MEDICAL INSURANCE CARD. I NEED TO GET MY NUMBERS OFF THE CARD AS I WAS SEEN BY MY DR TODAY AND HAD NO PROOF OF INSURANCE;PLEASE HELP
