How To Download Aflac Claim Forms

Go to the following Web site, where you can find the wellness forms for the accident, cancer, hospital indemnity, and vision plans:
www.aflac.com/business/business_services/claims_form.aspx
Click on the state where you live, then the choices of forms will show up.
Once complete, please fax to 1-877-44-Aflac for processing. Any questions, you can always call policyholder service at 1-800-99-Aflac, 8 am - 8 pm EST weekdays, for immediate assistance.

Hi Rwood, LeannCLevine is correct. Did you know that Aflac agents and policyholders can now submit supporting claims documentation online through Aflac's SmartClaim system at Aflac.com/smartclaim for initial claims. In addition, the easy-to-use browse and select feature is available for existing pended claims as well, through the claims history screen. Document upload is available for proof of loss for all Aflac policies except life and wellness claims. Text documents; .doc, .docx, .pdf, .txt, .xls or .xlsx, and scanned images .jpg, .jpeg, .jfif or .pdf are all accepted.
