Good Morning, I received a check from Aflac concerning an annual physical, however, it lists the patient as Janice and the patient was my husband Paul? Does the check need to be reissued as I will have my annual physical next week and would expect to receive a check for my visit also? Hi Janice,

Based on the fact that you were paid $60, I'm guessing you have a current personal accident indemnity plan with Aflac.

If that's the case, the PAI plan wellness benefit is $60 per policy per plan year. Therefore, you have already been paid the benefit for the current plan year.

The treatment (physical, dental, etc.) must be done within the current plan year in order for the current plan year benefit to pay. However, you may wish to contact 1800-99-Aflac (client service call center) to check to make sure you have no other outstanding wellness benefits, from previous plan years. If yes, Aflac will honor those claims as long as you can provide information regarding the specifics of the treatments.
