Why Doesn't My Balance Sheet Match My Final Reconciliation For The Year?

We are off by quite a bit. There were changes made as to which accounts some items were attributed to - what is the best way to find the discrepancies? Monthly our reconiciliation reports balance out to our statements, but year end balance sheet is not balanced. HELP
